Create 4 files:

1. LoadingScreen_MapName.tga

1024x1024 with alpha

Alpha Channel:
White = opaque
Black = transparent

2. OutroTitle_MapName.tga

1024x256

3. l4d_mapname.tga

256x128

4. addonimage.jpg

128x128
