Create 4 files:

1. LoadingScreen_MapName.tga

1024x1024 with alpha

Alpha Channel:
White = opaque
Black = transparent

LoadingScreen_MapName.tga 1024x1024 with Alpha

 

2. OutroTitle_MapName.tga

1024x256

OutroTitle_MapName.tga 1024x256

 

3. l4d_mapname.tga

256x128

l4d_mapname.tga 256x128

 

4. addonimage.jpg

Part 2 covers:

Using VTFedit to create VTF [Valve Texture Format] and VMT [Valve Material Format]

I used Photoshop.

But I believe you can do it with GIMP as well. GIMP is free and similar to Photoshop. I think it can open PS files.

Hey Man Your tut's rule!! They've helped me countless times! But what program do you use on this one? and Could you do a tutorial on water(lake, river, etc..) Thanks for all the help!!

Excellent tutorial as always. Your tutorials really have helped me to create my first map.

Cant wait to see parts 2 and 3 to be able to finish my map.

Btw, could you do tutorial about rain effect someday... seems to be impossible to find one.
